These forms have been developed from a variety of sources, including HIPAA and the National Medical Council, for use in serving our patients with the best and most thorough care possible.
There are forms for patient registration and records release as well as for a complete medical history. You will also find the HIPAA (Health Insurance Portability and Accountability Act) Privacy Policy and Acknowledgement forms.
For questions concerning self-pay, commercial & private insurance, workers’ compensation and Medicare please consult our Financial Policy form.
Lastly for treatment of a minor including; infant care, childhood development and adolescent medicine please see our Authorization to Consent to Medical Treatment of Minor Child form.
Additional forms may be required during your visit specifically for pediatric and infant care but these will cover much of what is needed for a standard treatment.
